 Sample 504 Plans for Students with Chronic Illnesses:

Here are links to sample 504 plans.  If you are creating a plan, you can use these as a starting point and change them to fit your specific needs. Even students with the same illnesses do not always share the same symptoms or need the same accommodations.

504 plans can be simple, including one or two accommodations, or they can be more involved.  Feel free to add accommodations that you don’t see listed in the samples.  There is no set list of’s really about working with the school to develop a plan that is reasonable and meets your needs.

If you have an illness you don’t see here, you can modify any one of these to fit the specifics of your illness.  Many accommodations are quite similar even though the illnesses are different.

I like to include a more personal letter to teachers with my kids’ 504 plans that explains their illnesses and how they affect the kids in school.  There are examples of these letters in some of these links, including those for both of my sons, Aaron and Jack.

    These samples of 504’s are great and I appreciate them. My daughter suffers from migraines and depression and I’m having difficulty finding information about 504’s for depression. Do you know where I can find some samples?
